This could go wrong in a lot of horrible ways. Simply being disintegrated immediately in transit would probably be the most peaceful way.
I think this might go on to motivate Alfred to try and 'bring him back', and those ventures lead to putting the world on a cycle of apocalypse. Either that or this experiment manages to do it.
That and running such a risky "small" experiment on a living (human or otherwise) test subject violates pretty much every ethical standard for scientific experimentation or trial possible.

Well if this is the origin of magics, it's interesting to think the magic to erase everything might have been made from a transporter device.

.....so......what's the worst case in this scenario? i mean, we have:
atomized
scattered across time and space
accidentally fused with a fly (or other similar creature)
have them end up in hell for what, to them seems like an eternity but to everyone else is no time at all and comes back as an evil demon slave
turned inside out and splattered everywhere
only partially teleported, cutting them in half
or split their atoms and turn them into a humanoid nuclear bomb
atomized

Hmm... "first live test" and she just straight up walks on? Kinda reckless if you ask me. You'd think they'd send something less... uhm, essential to the project? non-sentient?

I've just got this horrible thought: assuming the energy brought in to replace the mass brought out... Just, not converting the energy to proper mass (as mass and energy are equivalent)...
Even assuming that our poor ferret/weasel fellow there weighs only oh, say, 70 kilograms... Some fast math of E=mc^2 nets me...
299792 km/s ^2 = 89875243264
89875243264 * 70 kg = 6291267028480 kilojoules...
Or around 1.503648907380497 megatons.
One and a half megatons... We'll go and assume a ground burst here (under 500m altitude)
Enough to vaporize a decent chunk (1500 meters radius) of Manhattan Island (New York City, New York, USA), blow down buildings out to 2.5 kilometers (and likely kill anyone in that radius, too), with an air blast causing 50% casualties out to 5.25 kilometers, and give folk 3rd degree burns out to 12.7 kilometers (or, if you're super-powered, a really really wicked tan!)

And this, ladies and gentlefurs, is why we don't DO disassociative teleportation, and instead rely on macro-scale quantum tunneling: you're not dealing with good ol' Mr. Einstein and his nasty nasty equation.

You don't 'Engage' in a teleportation room. You 'Energize'. That's why it failed.
