The gelding (steer, whatever) is bound so that he can be flogged. The flogging is taking place. He is not screaming, or at least his screams are muffled. The gag he has prevents or muffles any screaming. That's the scene... but an inset shows this isn't just any gag. It's not a simple rubber ball-gag. No, it's a balls-gag. In fact, it's the set he used to wear rather lower and that used to be part of him. The inset might be a cut-away or pseudo-X-ray view, or it might show an earlier time just before the gag was slipped into place.

Note that this is having the parts used as gag. It is NOT "Now eat them!" She could do this to him over and over...
