Possible Surgery?

My bottom jaw has always been kind of...well not really there. Always pops out of place, and just makes my whole head look awkward and kind of deformed?
I'v had people comment on it before, and I myself hate how it looks.
So I was thinking of having a surgery a like to the one my nephew had. His was for his cleft palette, but mine would be for just reshaping and kind of lengthening it?
The surgery also helps to open the airway more, which would be fantastic for me..
Following are some of the conditions that may indicate the need for corrective jaw surgery:
Difficulty chewing, or biting food
Difficulty swallowing
Chronic jaw or jaw joint (TMJ) pain and headache
Excessive wear of the teeth
Open bite (space between the upper and lower teeth when the mouth is closed)
Unbalanced facial appearance from the front, or side
Facial injury
Birth defects
Receding lower jaw and chin
Protruding jaw
Inability to make the lips meet without straining
Chronic mouth breathing
Sleep apnea (breathing problems when sleeping, including snoring)

My only problem would probably be money and insurance...
I have no clue how much it costs currently, and I don't think I have health insurance right now..
I really want the surgery dammit..
