Escapement doesn't mean you escape.
This one is a bit complicated. The male is bound, with balls being squeezed in a vice. The vice isn't turned by hand, but by machinery. It's a sort of giant clockwork with an escapement that is activated by not by the machine itself but by the pulling of the victim. His wrist cuffs are connected by rope or cord to each side of the anchor lever. As he struggles, he releases the escapement and the machine turns and closes the vice a little more.
What makes him struggle and twitch? Why, the sting of the whip since he's being flogged. If he could just hold still... or still enough long enough... maybe.

And of course, I imagine there a bull.
It's almost disappointing at first.
Then a bit fun.
Then a bit much.
Then painful.
Then... if there's no gag, what will happen, and happen first?
A try at a safeword? "Ok, the joke's over." Pleading, begging, screaming, tries at negotiating, insults (how counterproductive!) or a stoic near-silence?
Also, it's sort of like a time-bomb, only you don't know when something will explode.
