MWFF 2015 Badge Orders Open! Order via Form

So I just uploaded this and it's that time when everyone's posting about it: MFF badges are up for sale!
I am opening 10 slots to start off with to make sure I can get them all done in time for the convention. I finally acquired a job (two, actually) so time is sparse, but I wanted to get some out for you guys!
All badges will:
-have the name of your character
-have the same background/framing
-have my signature on it
-come with 3 mil lamination, printed on 110# index cardstock
-be available for pickup at Midwest Furfest!
Please comment below with the following information:
They're priced at $25.00 USD each!
First come, first serve, on the condition that the form is filled out thoroughly and correctly.
