A month or so ago, I started really wanting to map out some of my characters' bios to get myself in the mood to do some creations and whatnot, but when I looked for sample templates online, everything was either complex to the point of the bio being longer than any story I could write, or simple to the point where I could've figured it out myself. So that's what I did. I created my own character template that I took great pleasure in using on a couple characters (whose bios will hopefully be uploaded in the next few weeks). I hope you find it as useful as I did.

A few notes: if there's categories you don't want to fill in, that's fine. If there's categories like relationships or family that you think are too small for your specific character, feel free to expand. If you do use this template though, I'd love to see what you came up with, so link it in the comments for me to read.
