If hearing is critical, then definitely would fully stick out. Think of a military tactical helmet; better to lose an ear than your life. Additional/special protection could also be included, but that would likely be tailor-made for the situation/usage.
Same could also be used for just normal fashion head dresses, like caps and the like.
If hearing is important, but not critical, then probably a thicker helmet so the bottom of the ears are protected, but the floppies could still stick out. This is where I would classify the bike helmet; needs to absorb a shock, but you still need to hear your surroundings for vehicles and the like.
If hearing is not important, then fully encapsulated within the helmet - that'd be one thicc helmet, though. Because you wouldn't want bumps or extrusions, as this would create tiny pressure points, which could more easily compromise the integrity when "in active use".
But that's me; I don't have an engineering degree or anything. I just know a little bit about way too many things, and I just extrapolate stuff. XD
