The past week has been particularly challenging. My father, a Navy veteran suffering from PTSD and Lewy Body Dementia, experienced a fall resulting in a broken hip, necessitating a two-hour surgical procedure. Following surgery, he was transferred to a different hospital for recovery, strategically chosen for its proximity to home. Upon his arrival, the medical team reviewed his medication list with my mother and inquired about his current prescriptions. Notably, one medication, which had previously been discontinued due to its potential to induce psychosis, was mistakenly administered at full dosage despite my mother's explicit warning regarding its risks. After expressing her concerns and repeatedly requesting access to the medication list, she was eventually provided with the information, allowing her to identify the error, which led to the immediate cessation of the inappropriate medication. The medication in question requires initiation at a minimal dosage, which is gradually increased to achieve therapeutic efficacy while monitoring for side effects. It is crucial to adhere to a slow tapering process when discontinuing the medication to mitigate the risk of exacerbating psychotic symptoms, as abrupt cessation may lead to irreversible consequences. This careful titration is essential to ensure both safety and the effectiveness of the treatment regimen.
He endured five days of both auditory and visual hallucinations alongside his Lewy Body Dementia (the very reason he was taken off the med to begin with). On his initial night in the hospital, he fell from his bed, and it remains uncertain how long he lay on the floor before staff noticed; it required insistence to obtain an X-ray to rule out complications from his recent surgery. During the second night, he experienced another fall, which postponed his transfer to the VA Home by one day. Currently, he is at the VA Home to continue his recovery and commenced psychic therapy today. Since his admission to the VA Home, he has fallen out of bed three times during the first three nights, prompting prompt staff intervention and communication with my mother. With the psychosis stemming from a medication error—an evident case of medical malpractice—now subsiding, he has not fallen again; however, it appears that his LBD symptoms may have exacerbated.
