Just updated my TOC journal...which can be found here...

These are not complete, and are constantly under revision. As such they are both guidelines and set in stone.
I would like to point out that my commissioners have all walked away very happy with results, and I have had absolutely no issues with any commissioners to date. These TOC's are simply to protect myself (as some friends have suggested I do due to hassles they've encountered) and to let my commissioners know how things tend to work :)
