Honestly, some commissioners on FA treat artists like shit (you know who you are, commissioners!) by having some rule about how some puff of hair must look like, and where some piercing must go proportionate to his dick with almost scientific calculations and stuff. If you want that, go to some architect and see of they can make a technical blueprint of your character! Otherwise, I'd suggest to go someplace else with your stupid rules and regulations on how YOU want it with your bizarre rules, instead of the other way around! You've seen the work of the artists, don't expect them to do the impossible, byt commissioning something that's out of their comfort zone, or simply impossible to do.
YOU commission them by THEIR rules, and if you don't like it, you can go take a hike! You should be thankful that artists exist, not completely drain them from their creativity, and above all: you will have to understand they have lives too, and are not simply art-making machines for you to shake dry, just to statisfy your nonexistant sexyness in the form of some nonexistant character.

In regards to getting details to a T, that is basically impossible for just about any artist to do, since we can't see into the minds of the client to see exactly what they mean. It is a little appalling when a client will not ask politely, but -demand- that there be made changes to a silly, inane detail that shouldn't really matter in the first place. They -should- realize that an artist has certain strengths and certain weaknesses before commissioning by surveying their gallery. After all, they are commissioning the artist because they like their art, and shouldn't interfere with the artist's style by trying to change something or get them to do something they wouldn't normally do. It's like asking someone to speak a language perfectly that they have never spoken before.
There -are- grounds for asking for changes though, don't get me wrong. But there is a point where it gets ridiculous and out of hand, as you say, like with having a perfect amount of puffiness to hair or the perfect cock length in proportion to something else right down to the centimeters. An artist's TOS is not necessarily meant to limit someone, but more to protect themselves from getting ridiculous clientele interaction to the point of over stressing and just simply spending too much time and effort on something that shouldn't be an issue.
As an artist myself, I would be willing to take it so far as long as it doesn't get out of hand, but only when respect is mutual between both parties. If the client doesn't respect me, I am not going to deal with them anymore and simply give them partial/full refund for the work that has/hasn't been completed, because at that point it's not worth my time and effort to appease someone who is disrespectful and unnecessarily picky about superfluous details.
