Just download and open it in sai or photoshop (not sure which program you're using but should work for both), take out the layers you don't need or want to edit, crop some of your artwork you want to use as examples to fit into each area and then edit the rest to your liking.

Again, it's simple and you probably want your own custom price sheet but it's a good one to start with *shrugs*

If you let me know what images/text you'd like on it and a color scheme I can whip one up. I should have internet back today. :)
