Well I would say look around how a lot of other artists do they layout for their journals when they offer commissions.
but usually it breaks don't into first of listing what sort of styles you offer (sketch, colored sketch, lineart and different forms of coloring are the most usually onse) together with examples for each style and a listing for the pricing as well additional costs like extra characters, backgrounds props or very detailed stuff.
In regards to pricing you have to see how much time you put into each thing and what you yourself would consider a good hourly wage for it as well as maybe considering what you feel a pic from you should cost.
last thing is usually the do's and don'ts that you draw which is stuff like only safe for work stuff or if you draw smut/porn/not safe for work stuff as well as maybe possible exclusions in regards to different fetishes (vore, gore, hyper, cubs and so on).
then maybe a good additional thing to have in your journal is a list over the people that you are currently do commissions for as well as maybe small status updates to it, which could include if its already payed or not, if your sketching it right now or coloring it already.
