Is there any way for a humanoid inflate not to make everybody go "oh it's a blow-up doll!"?

Yes, yes there is... You need to make it undeniably clear that it's not a blow up doll and either block and/or ignore anyone that doesn't take the time to read your actual description. I had a few problems with people mistaking my characters as sex toys until I took a stand against it, don't worry about seeming like an asshole so long as you give them a few chances first or you're going to have everyone walking all over you.

Eh, I was thinking more in terms of design.

in that case I'd suggest that you try to keep their design proportionate to their size and not focus on the genitalia and maybe not have any visible as well as making it clear that they're not sexual characters.
