i dont see why it should matter with the police?
Doctors aren't the only people that play into healthcare. If people don't need to know, they don't need to see your identity information at all. If they do, then it's likely for identification purposes that may be necessary for the police or in a worse case circumstance, healthcare.

Because police need to be aware of medical situations and biological identity checks.
Sure, potentially the police and healthcare could be dealing with it and by extension, anyone who would need to provide that information to them because they could be involved in the situation too.

One of the most reliable markers out there is the sex of an individual. Reducing the amount of reliable genuine identifying information makes it harder for law enforcement and healthcare to operate effectively.
That said, I cannot really think of any government papers that don't request identifying information like sex unless it is additionally kept for police and medical purposes to go through later (such as someone's driving license).
Perhaps a solution would be to provide two fields, one called 'sex' and another called 'gender'. That way people can make it clear what they identify with and the police/healthcare people get what they need.
