I like the form. It's quick, easy to navigate, and doesn't mean you have to peruse a template to figure out what the actual cost will be.

One suggestion, though; the text in the boxes could be, I believe it's called "representative values". It's the sort of text that shows what the form/box expects(I.E, the name box expects a name), but it's automatically erased once you select the box. So you don't end up accidentally writing, for example, "FluffythingKaedal" in the name box. It's a small detail.
