Well, that usually happens if you are leaning to the side when drawing.
Do you not notice it until you look at the picture straight on?

It could be that you're using the side of the pencil/pen. Or you're leaving your wrist laid flat as you draw.

Do you draw with your wrist? Or do you move your whole arm?

It helps to see any mistakes like that if you flip the image in PS. Or hold the drawing up to a mirror. (I notice my drawings leaning all the time when I check doing that XD)
And to draw circles, it is helpful to draw a line first and then draw the circle around that(easier to see when it's split in half) :3

Oh, the first part is if you draw things leaning a bit and don't reslly realize it at first. Disregard that part if it is something you do see right as you are drawing it x)
