Icons...?

I have a question regarding icons. o^o
How do people make them so well? Do you just make a piece like normal, in whatever scale is comfortable, and then scale it down, or...?
I've been wanting to draw a new icon for a while now. xD;; I'd like one with my sona on it. o3o One that looks pretty legitimate, and professional. I can never seem to make it work, however. xD I guess I just need to keep on trying, and eventually I'll have something I like. =w=

So i guess that's the norm for most. ^^
Or draw something simple small, then scale it just a little bit to fit the dimensions.

I've also seen a lot of people use a 100, 200, 300 pixel square piece of a larger image and downsized that.
If the quality from downsizing is too poor, drawing in a 100X100 picture zoomed out can help to orient you better. I would know, I tried and failed miserably drawing one super zoomed in.
The most depends on the program, what one do you plan on using?
I'll be using Autodesk Sketchbook Pro 7, because I don't have anything better. xD
