Comment trouble on submissions

How come most of my submissions the "add reply" boxes are off to the right hand side and people can't comment? What gives FA? What's the story? Am i suddenly doing things wrong? I thought we were cool with each other.
I thought it was just a bug or a coding thing that was getting worked out and I mean I doubt I missed on much attention but it's been a while now and it's sort of irritating me.

This as an example the comment box and all the smiley faces are on the right hand side and the comment box is squished. And I should say that I've had this before and had it gone away on the submission and people were able to comment but it was after some time.
