Media: (Sketch or Lineart or Colored or Full Color.)
Commission idea: (basically a brief summary of your request)
Details: what the commission will be about.
Actions: (what are the character(s) doing)
Poses/ positions for each character.
Expressions: what are they reaction to.
Background: Yes [simple or with more effort] OR None.
(if yes, tell me what you have in mind for a background.)
References: (for characters, poses, clothes, objects, etc.) Always bring reference images in a note, if there isn't one, it will not get through.
