I'm Rude?

So time and time again, my co-workers and I are told that we are rude. This comes in the form of phone calls, 1-800 calls, and Satisfaction Surveys. And against this, there is no defense.
Just five minutes ago, a man comes in. I smile and ask him how I can help him as he is pulling a coupon out of his pocket. He proceeds to shove the coupon so close to my face that I lean back out of instinct and I still can't read it without looking at it cross-eyed and goes "Double Cheeseburger, extra pickles. Nilla shake." I successfully keep my composure and go "What size Vanilla Shake would you like, sir?" He proceeds to shove the coupon at my face AGAIN and goes "I said nilla shake!" I take the coupon from him and it specifies small, so I complete his transaction, make his food, and hand it to him. Without a thank you or anything, he turns, walks again, and says "Gonna be one of them days, you dumb motherfuckers can't fucking listen."
So again, my thought here is WE are called the rude ones?

he demanded a cheese burger with out the cheese
so I said you want a hamburger
he screamed no! I want a cheese burger with no cheese
so we gave him a hamburger and he was fine

Which begs the random question, why is it called a hamburger when it clearly uses beef? Is "Beefburger" not catchy enough? I've always wonder that.

How illiterate is he to not read the "Va" in "vanilla"? Did he legit think that the "va" means something else and the actual word is "nilla"? Or the better question, what drugs was he on!?
I remember when I worked at McDonald's a few years ago, some guy in cowboy gear that looked like hadn't taken a bath in god knows when he starting yelling and cussing at my co workers at the cashier for no reason while throwing random cash from his wallet and DEMANDING food right at that moment. The cashier told him to stop it or he would call the police, but the manager (which didn't even know what was going on) FIRED the cashier. I went up to my co worker that got fired and I said, "I hope you're going to be okay because you did NOT deserve that."
Also, he said "'nilla shake"? This guy sounds like a redneck trucker or something. ^^;
No, you aren't rude dude, as I said it's just people are jerkass idiots.
On the other hand I'm not an obnoxious and rude customer making your day ten times worse.
On the third hand you don't report people for being "average" or "very polite and professional," you only report them when either you or they are really obnoxious and rude. So the only reason why you're only getting those may entirely be because of your customers, not you. In fact in might be just one dude over and over again. Which would be horrid.
