One step at a time. I don't know what happened to spur this, but to at least lessen the load of things to subject yourself to self-degradation over, one thing at a time.
Pick one, and do what you need to do to fix it. Like... Just try and brush your teeth at least once a day. It's not much, but it's one thing you can do looking on a small scale. I picked that one as an example as my teeth suffered from my own neglect and it's still all I can do to brush them so often, and I'd like to at least help you pick out something that can do more than just self-esteem issues.
