Made a form for commissions!

I was never a huge fan of forms for this kind of thing, BUT lately I've struggled after being contacted via what feels like 10 different means,
as it makes it super hard to keep up and find everything and kajhfkjshg
SO I felt if people just submitted it the same way, regardless of where they contacted me, it'd help me a lot. I made a form ages ago actually and just spent like an hour revising it!
I'll try to post it for people to see, but I'll likely just link it to anybody who messages me about a commission.
