COMMISSIONS OPEN! 5 SLOTS (READ BEFORE SUBMITTING A FORM)

Okay, So an update; I got my laptop back, but not the data. So basically that means that backlog commissions will be on hold until the data comes back., however I do have my laptop and a 500 gig SSD installed on it, so that means I can still work, just not on the backlog.
This means I wanna try something. I'm gonna test out this new commission form that I've brewed up. This form will ask the questions needed and I'll get back with the customer. Basically there's no way I can lose the information in the form and I'll look at the form for reference on what the customer wants. Making it way less of a hassle to go back on messages and look stuff up. (I might even try to have previous customers submit on it when I get my data back)
But that's beside the point. I'm opening up for 5 slots. These slots will be completed within 2-3 weeks.
Another thing is that I will only be accepting one sequence commission per batch if any come up. This will lessen the workload and time crunches.
The prices are included on the form as well; I'll be taking the first 5 forms and close up, but my prices are also here!:

These are technically emergency commissions since my data recovery bill is gonna be pretty high as well, but regardless if you wanna order, Go on ahead.
