Please give this form to your parents, do not fill this out at all, your parents must read and agree to all terms before signing and turning this form in. All updates and questions will be asked through your parents. If your parents do not want to go through with signing and or filling out this form then we cannot allow you to: send us money/try to commission us yourself/ use a parents card or cash for payment (<~ESPECIALLY NOT THAT) We do not want to have to deal with any type of legal issue or money issue if something like that comes up. EVEN IF YOU ARE UNDER 18 AND YOU ARE PAYING WITH YOUR OWN MONEY YOU STILL NEED TO HAVE THIS SIGHED. No hard feelings it's all just making sure that you guys, and us are safe ^~^
