About the Image:
Well I didn't put a mature tag on it because the blood is only a metaphor and I just don't think it's considered gore. Anyways he's basically changing himself to conform to "society" so that he can feel normal and finally fit in, but in the process he's destroying himself inside, "killing" his true personality. SO the blood is more like he's bleeding on the inside because he actually has no wound and is his own blood, no one Else's. No he didn't kill anypony.
