The insult comes in the form of "Your identity as ____ gender is misguided and not worthy of respect, I know better than you do, and hence will call you what I think you should be called."
To say that people are getting upset because they think the gender they are being called is "lesser" isn't really true.

When society tells you repeatedly that you are lesser because of your gender, when you experience dysphoria on a regular basis, when you are told things like you are a monster and unloveable because of what you are, when people constantly refuse to recognize who you are, hopefully it's understandable why "being confident in one's gender" isn't the issue.
