I mean it is not that hard! The point is to know what variety of chords can follow the one that you are playing at the moment. If you know the progressions, it is a good help for you to never stop in the middle of improving.
The worst thing is to combine left and right hand non-stop and you should think of 5 different things at the same time:
- Right hand - Melody (in this case)
- Left hand - Harmony
- Pedal - to sustain proper chords and not to mix "Water with alcohol"
- Harmonic progression - what accent do you want to put, what is going to be next.
- Remembering what you playing - because if it looks like a theme, you will or should repeat it twice or triple it during the impro.

Yeah it actually seems hard...

You explain things better than my teacher. Maybe if you were my teacher, I would have talent when it came to piano.
But, alas, you were not my teacher.
