To achieve high altitude flight or flight for extended periods of time a dragon would need it's main lungs and several pairs of air sacs which is common in avians.
Birds respiratory rate is slower than of a mammal of similar weight yet they can get more oxygen with every breath because of a more efficient respiratory system.
