Anatomy-wise it would most likely converge into one below the collar bone, three going down to connect to the belly would take up space needed for other organs and have a chance of blocking the tube next to it.
The reaaal problem is when you think about how he'd breath! Cause three windpipes connecting to the same lungs would constantly be battling to take a breath or let out one! Unless of course the two side heads only had an esophagus and the middle head was in charge of the breathing, or each has a trachea and the middle brain is the only one that subconsciously controls the breathin, OR each had everything and all three brains would get the "took a breath in" signal and react at the same time to take regular breaths XD

Now that you've given me info to think about, IF I were to design Cerberus anatomy and how it worked, I'd say 3 trachea, but only the center one is required to breathe. The two outer heads only use their trachea to exhale when making noise. Growling, barking, howling, etc. They COULD be used to breathe in, if the main head is busy, but by default, the center head is the primary breather.
As for brains, I think they'd be like a hive mind. While able to process into separately, they work in unison perfectly, so rarely do the brains trip up over each other. Think of some of the conjoined twins who share most of the same body. While struggling to learn to use their shared parts in youth, by the time they are adults, they work together as seamlessly as you or I with our one brain.
I imagine, since you said the 3 esophogi converge into a single tube, rather than 3 stomach entrances, the 2 side esophogi would have entrances like you or I have in our actual throats; a closed hole that only opens when pressure is applied to it, such as when we swallow, be it solid, liquid, or even air.
If the Cerberus is intelligent enough to speak, surely this means one or more heads can converse while another is eating. Or if they are a playful pred like so many of your characters, they might toss/share a single victim between each maw; savoring the taste and teasing the victim before one of the heads finally swallows.

Also speaking with each other wouldn't be too much of a problem if they did work together perfectly, since a wider body might get enough air to let two of them talk with the same breath. I agree with the breathing difficulty issue, and feel that your solution; that the side heads are one way [primary drawing in] whereas central head is mostly out,
Alternative they could have more complicated pulmonary systems' since each head would need a large oxygen supply for each brain. Perhaps there are multiple hearts ?
