After countless ages without a material form, coming out of Purgatory and having your soul adjust to one again can cause quite a violent reaction. In Heaven's case, such a long time in Limbo started to accumulate on his soul a 'backup' of filth. Such is the effect of the place, it slowly creeps in like a disease and builds up like grime, rendering him sluggish and listless while he is trapped there. Because it was such a gradual change over many years, he didn't notice how bad it was until he was ripped back into his body, and suddenly all the grunge started expelling.
