So I picked up Far Cry 3 for a song the other day, and I did the first outpost-clearing mission like 4 times because I wanted to do it perfectly and get 3 times the XP. I didn't know it's basically impossible that early on in the game. The third time I almost completely aced it, just had the dog left… turns out you can't do a stealth melee takedown on a dog, right? Ruined. 4th time, the dog somehow sees me through a wall and I panicked and pushed the grenade trigger instead of the gun trigger. Yeah.

The worst one for me was Deus Ex: Human Resources … must've spent 2 hours going through one area, because it wasn't enough to get by undetected. No. I had to get a perfect non-lethal melee takedown score. And man, I did it, I really did. Except for the stupid boss-fights. Anyway, got right to the end of the game, pick an unsatisfying ending from the ending-vending machine, credits roll… where's my you-didn't-kill-anyone achievement? Nope, apparently about halfway through I knocked someone out and they drowned in a puddle or something. Graaaaahhh!
