Ok, this is the very start of my pocket watch, I've constructed the top half, and part of the chain. As you can see, I gutted a real alarm clock to have working gears show through the top, they have nothing to do with telling the time, they're only there for effect.
The small hole is there so that when constructed the winding bit can be stuck through and reattached.
