My first Skyrim character was a male Khajit named Thirteen, and I went vampire lord with him. May he rest in peace, though. I had to get rid of his file because it was soooo glitched out (my first time ever playing skyrim and I had all the DLC and stuff so I ended up doing stuff in waaay wrong order >__<)

Yeah! It comes from quests that are labelled as "Radiant" quests- they're basically quests that randomize what location you go to.
Well... if you've already cleared a location out (i.e. looted it fully and killed whatever "boss" was there) you can fuck up certain questlines that include radiant quests.
My advice- don't go anywhere you don't HAVE to go to. If you see a named item, DEFINITELY don't pick it up UNLESS the quest you have selected is to get that item.
I've had sooo many fuck ups- most of them involving the Companion's storyline and then the Dawnguard DLC.
The fun thing is, if you do it right, you can actually be a Vampire Lord/Werewolf hybrid! But yeah, getting to that point is a royal pain in the ass.
Unfortunately, glitches like this plague most of the Bethesda games. Fallout has tons of issues like that, too. There's just so much shit to DO in those games in terms of main quests and sidequests and all the sheer amount of stuff that sometimes they just didn't get the coding right, and even with the patches that skyrim has had, it still has issues.
