Basically it's a French tradition : in the cake you have two hidden items one being a small figure and the other a broad bean. Whoever ends up with the small figure has to buy a cake next time and is crowned king, the person who gets the small figure has to buy a bottle of wine for the next time ;) ( sometimes the rewards switch ).
Here where I live ( France - yes I know an italian guy living in france is weird ) this kind of tradition is followed by a ton of people and bakers do follow it eagerly. It's also a nice time for collectors as those small figures differ from baker to baker but you can get anything really. HTe most common ones are small figures linked to christmas ( the three Mages, Joseph, Mary, the baby Jesus ) or just remotely linked to it ( the farmer, the blacksmith, the baker,etc...) - that tradition inherits a lot from christianity to begin with - but it can also be simple landscapes from different regions, or even famous buildings and in the kid's versions of these you can even have modern figures ( like Spongebob :P )

Little bit of trivia:
In France, (where this is celebrated), it's an etiquette faux-pas for the President to wear a crown. (because, you know, french revolution etc.)
So, at the Elysée, on kings cake day, the pie on the president's table is required by law to not have the bauble inside it.
