For those who follow Midnight Dew is the foal Fluttershy said from the pregnancy caused by the MPL doll. The reason for the near look alike is due in part that doll was not outfitted with any DNA from a would be sire, instead it used Fluttershy's own to make the foal.

she did lose the doll, but after she was already pregnant from it. She then went through a period where she was a wreck emotionally cause she was pronounced infertile by the doctors yet the doll helped her bear a foal. Upon losing it she had lost her ability to motherhood again (she yearned for a family, more than just one foal). After time with discord he was able to mess with her to get her pregnant again, with a few affects.
Thanks, though fixing that for a pony that is pregnant and meant to show the belly off is more difficult than it seems. As for your question, either could work. I personally like the idea of her being pregnant for a full gestation at that size. Even with its size it feels comfortable to her rather than a burden but she knows well enough how it feels so she is getting the benefits of having her pregnancy nice an full with limited drawbacks
