I start out with something called Zspheres and do a rough gestalt, convert it to polygroups and dynamesh it as I go.
The neat thing with zbrush (since 4r2 anyways) is that it has an echorche anatomy figure in the tool menu, it looks like this

I usually take her out and eyeball her and compare here with my own sculpt, trying to fix too glaring anatomy errors.
