Cintiq tablets are those tablets that have a screen that you can draw on: http://uk.shop.wacom.eu/products/cintiq
But obviously that's silly, why would you want to obscure thE VERY THING YOU'RE DRAWING WITH YOUR OWN GRUBBY HAND.
So clearly, this must be how Cintiq users use their mice and obviously want their tablets to work the same way.
On an unrelated note,

Clearly, you'd use a separate screen that you keep turned off, so that the surface is the same color and doesn't mess up the laser. It was much better with a ball mouse and glass screens, you didn't need this.
But instead of a monitor, it's usually cheaper to use a large, single-colored plastic surface that sticks to your desk.
Actually, that sounds like a business idea. I'll call it mouse pad.

I do, having used my DS for a lot of sketching but then moving onto SAI + Wacom tablet.
Being able to draw on the screen directly was great, but for the sketching process it meant a lot of faffing around with a low resolution canvas, low processing resources and other limitations. I had to give up being able to draw on a screen to make way for far better drawing resources.

But ideally, you'd want to have your arm in a neutral position going downwards from your shoulder, otherwise it gets tired from holding it up. I don't know how you'd put your tablet that way while laying down, since virtually everything in front of you would make your arm go up :P
Turns out you can draw while laying down: https://www.weasyl.com/submission/69196/
Doesn't look too comfortable to me though :P
