Nonono, Makoto is fine! The art is basically represent Ginataro inner feel... He may look fine and all but he actually feel lonely but with Makoto around, he feel less lonely and have fun with her. Since human not live forever, this possibly close how Gintaro feel when Makoto pass away. This is just how I see through Gintaro's feel, this not happen in either anime or manga.
