That's the wall, after you leap over that it isn't difficult at all. It is however time consuming as hell and you can easily blow through a week just poking and prodding a single sculpt. XD
I also have a few materials specifically for fleshy things (gums, open wounds, tongues etc.) you can use/try if you want. =)

Oh no, the alphas are overlays. The materials are the actual materials the objects you made are made out of. That changes the way light and shadow react to it.
Here are some I downloaded a while back

just unzip these and put them in the ZMaterial folder. Don't recall if there was another step to making them show up, but that's where they go.
I also don't recall if they are all useful, but I do know I wouldn't have been able to make that synx without SkinShade4 and SkinShade5; I swear by those two. Those were basically the two materials I used for the outside skin and the inner/mouth skin. Also the eye reflectors, for example, are specifically for eyeballs and making light reflect off them like real eyes, etc. It's been a while since I really used Zbrush, so you may want to look up a tutorial on youtube about using materials if you don't know the full ins & outs. My memory is fuzzy, but I think you can just paint them on as well. @_@
