I think Bethesda was a bit sneaky with it claiming it is a free to play game with optional lunchboxes purchases but fail to mention your dwellers will die without constant 24 hour supervision hence the addition of the Mr Handy robot that you have to buy to collect resources while you sleep to keep your dwellers alive and happy.
If you exit the game, like actually open the options and return to the main menu, then things pause... sorta. Every resource room will continue to accrue til ready to harvest and anyone out exploring will keep exploring, but resources aren't used and you don't get any events like raiders or radroaches happening.
