To be honest, this reminds me of an idea one of my friends had once.
That being, do an ENTIRE episode with House replaced by a Muppet, but otherwise, completely normal- everyone going about their business as usual.
And at the end it turns out it was related to some obscure symptom of the patient's. That or it never gets explained, which might well be even better.
