Sans was also smarter in battle by avoiding all of your attempts for an attack until the very last one. Regardless, I think everyone can agree that the genocide option really hits your heart and soul very hard, especially if you beat the pacifist side of the game first.
Well, considering how he was biding his time for the whole game to make himself hate you more and more by just watching you be a relentless killer and not stepping in, he became able to do so much damage to you not because of stats, but his emotional state. The snowdin library has a book that details that the more powerful someone's killing intent is, the more pain they'll inflict - and unlike every other boss except for undying undyne, sans truly wants you dead, to the point that he's just trying to make you quit, or his real goal, make you reset out of anger just to not deal with his power. Papyrus is incapable of killing you because he simply won't do it because he's too good-spirited. Even Asgore can't kill you if you're not at 1 hp, because he's not filled with hatred.

Well, I still find the genocide playthrough very sad and heart wrenching. As you're playing, you don't want to do what you're doing, but at the same time, you do, because you want to see how much is different through the game in comparison to the pacifist playthrough.
