And Salem knows what that means for her for the next couple of hours for her. After all Salem knew what she was getting into when she willingly became Karma's sexual outlet.

....It's basically a sexual symbiotic relationship between the two because Salem gets the sexual thrill she always wanted of being a pet and not being in charge but instead being a sexual pet to someone that would love her and care for her and give her a roof and warm bed and food.

Karma gets a ready and willing sexual partner, and because of that her disability isn't nearly as crippling as it once was (her disability is that her heat cycle never ends. Constantly in heat.) With Salem by her side she can go outside for walks and shop in stores and go to movie theaters. That being said though, Salem literally has to be by her side at all times outside because of how often her heat affects her and her mind if gone unquenched for any amount of time.

Also something they both get in this symbiotic relationship is a lover that loves them and someone that won't judge them for who they are and what they need be it sexual or just emotional... also on a bit of a side note even if Karma didn't get the money from the doctors, Salem would still be with her. I say that because Salem started dating Karma before she learned of the money and even now is still unaware of how much money Karma got, all Salem cares about concerning the money is that it is enough for them both to live without having to worry about being strapped for cash. Karma has offered to tell Salem multiple times how much she received, but time and time again Salem responds with the same answer "a pet should never know her mistress's funds. I am happy to just be here with you Karma."
