There's a story from Ypres, where supposedly, a Tank commander went insane and started shooting the engine of his own tank because it wouldn't go faster, while there were shells falling all around and bullets slamming into the hull. I really can't say for sure if its true or not, but given the immense stress of battle in this unproven weapon, it sounds like something could have happened.
