well, nephalem origins and their portrayal are a bit different depending on the source, i suppose... but usually they are the offspring of angels and demons, from what i've seen.
basically, they're angel/demon hybrids and are typically extremely powerful beings.
some examples are the main character from diablo 3, as well as the four horsemen from the darksiders video games.
