She's -fired-? That could only work if she was a company nurse, and from my admittedly limited experience, usually companies contract out to healthcare providers or clinics to provide things like health screenings, immunization shots, etc.

Yeah, while companies might be able to have a 'company nurse' on the payroll, all the hoops they have to jump through to stay in compliance with all the HIPPA laws while having employee medical records filed on the premises make it much easier, liability-wise, to contract with a local clinic to handle all the employee medical needs and evaluations beyond basic First Aid.
Even a clinic contracts out to a different clinic for employee medical evaluations and care, just to avoid the extra liabilities under HIPPA.

The short version is, outside of the other reply above, that some companies structure their healthcare management contracts with human resources, which may have the discretion to switch out the provider's representative, or even switch providers if there's enough discretionary power, though usually that's with board approval. Often it just depends on the structuring within the business.

the shark's job was not to evaluate whether a medical exam is necessary.
therefore, she was in insubordination, and refusing to do her task. i.e. not doing her job. plus she really has an attitude problem and a problem with authority.
