I wrote a program in QBASIC that lets me set up 'instruments' and then arrange them on a timeline. It's rather limited, no more than four notes play at a time, 64KB data maximum (Actually a bit less.) and a few other things. I'm too lazy to improve it now.

Indeed. Specifically a Yamaha YM3812 "FM Operator Type-L" (OPL) chip. Sadly to convert to wave and mp3 I need to use the NAME emulator. Again it's not exactly MIDI, MIDI files can do a number of things my setup doesn't allow. (Again, like being more than 64KB.)
