It's free if you're willing to play on an unofficial server. The official one went pay to play about five years ago or so. There are lots of free servers though. I'm on Atomix.
Mostly the amount of lag, custom quests and items depend on each different server, and how quickly the new updates get installed.
The pay to play servers that work with Gravity tend to be less laggy and have the latest game improvements the fastest.
The free servers vary wildly on server speed, number of players, and what kind of events they run.
