Here we have the officers with their names corrected as well as other officers I added in. Heres some things I found or noticed thanks to the following FA Users: kpfoxpaw, Kamu, edu2703, and RJfan.
-Trunkaby is Francine's Last name and The elephant I thought was Trunkaby was actually Bob Trumpet.
-Grizzoli turns out to be the polar bear, as stated by Jared Bush as well as the Zootopia Crime Files game.
-Snarlov is the white wolf despite getting up from the name Grizzoli. Since "Snarl" means aggressive growl with bared teeth and most dogs and wolves do this, not bears, it would make sense for the name to be the wolf's.
-The bear on the bottom right is not seen in the movie but on a wall in the Zoology Characters on the special features of the movie. His name ended with "mmers" and the first two letters were blurred a bit but I able to make out Simmers.
-Johnson, Andersen, Jackson, Krumpansky, Pennington, Stevens and Higgins are common names shared by most of the ZPD officers since, realistically, they weren't going to make every Zootopia characters unique.
So thats that. I also went ahead and made them look like small cards or ID Badges for your viewing pleasure. ^^ Hopefully this helps.

According to jared bush and rich moore, originally Fangmeyer was the wolf at one point but changed it to the tiger. http://zootopia.wikia.com/wiki/Officer_Fangmeyer
That means Delgato would be the Lion http://zootopia.wikia.com/wiki/Officer_Delgato

Its odd, since it makes sense that Francine's last name would be Trunkaby, since she is apparently the only elephant in the room and Bogo calls an elephant by that name... but her nameplate says her last name is Pennington.
Its dim in this pic http://i.imgur.com/6TnCOS0.png but you can see that its Pennington.
The question is whether to go with what Bogo says and that she's the only elephant in the room, or her nameplate.

Yeah, they must have forgotten to change her nameplate during the production or they initially was going to name her, or at least her last name, pennington. I will go with Bogo on this one than the name tag. Pennington would probably be for other female elephants in the movie. You can see the same name on another elephant near the end when they bust Bellwether. ^^
Sorry, but Snarlov is pretty clearly one of the polar bears. He gets assigned with Higgins (the hippo) twice. First to the Tundratown missing mammals cases along with Trunkaby, by which time the three of them are the only ones left in the room besides Hopps, and again at the end when they are both put in the undercover unit along with Wolford.
The one identified here as Snarlov, meanwhile, is seen answering to either Grizzoli, Fangmeyer or Delgato twice.
Short answer is that the team screwed up and let the aforementioned trio above be depicted as two cats and a wolf (this is not a misunderstanding -- the same three officers respond to those names twice), tried to fix it when they realized Grizzoli clearly implied bear, but wound up screwing it up even further by assigning the name Fangmeyer to the tiger, leaving the wolf with two possible names that do not match his species.
Shortest possible solution? *shrugs* Assign "Grizzoli" to two characters: the polar bear from the game, and his adopted wolf son who also became a cop, I guess.

True, when Snarlov is called the Polar bear gets up in both the beginning and ending of the movie when given their assignments. Its just that the names Snarlov (Polar Bear) and Grizzoli (Wolf) would make a lot of sense if it was switched with their species like how I did here. I mean, one of the people who made zootopia already stated that Grizzoli is the Polar Bear. Perhaps they just got mixed up with the names for these characters during production and the finished product may or may not be correct.
That would be a funny scenario if "Grizzoli" is given to both the Bear and Wolf. I could imagine them getting confused at who's being called and the odd relationship in between XD

Having so blatantly tried to change Grizzoli's species after the film's completion, it's pretty much the only option they have left to try to have a correct and coherent ZPD.
Personally, I think it'd be a nice little side story to visit in spinoffs or sequels: Mr. & Mrs. Grizzoli have a cross-species marriage, decide to also adopt out of their species, and the little wolf Grizzoli grows up idolizing his mom, decides just like Judy that he wants to be a cop, works his ass off, becomes top off his class, and later, top of the entire department.
His mom? Sure, why not? They identified Fangmeyer as female even though there was nothing by her appearance to suggest she was female. Maybe Grizzoli Sr. is actually female too (heck, she could even be the drill instructor).
