There's no set rules for it, the price is the same if I accept but I may simply refuse if I don't feel like having the character in whatever situation the client would want him/her to be.

Nope. But that list would include vore, inflation, scat, guro, probably some other things I can't think of right now.

Hm. I guess not, all those seem fine altho I've no experience whatsoever in transformation work and muscles are only fine as long as it's reasonable (as in "not Huge Quest").
